Myoglobin (MYOglobin, MYO) is a small molecule protein that stores and distributes oxygen in mammalian cells (mainly muscle cells). It is composed of a polypeptide chain and a prosthetic heme. Its molecular weight is about *8kD and about **3 amino acids Residues.
MYO is not a heart-specific marker, but it releases faster than CK-MB or cTnI in myocardial infarction, and myocardial damage can be detected 2 hours after symptoms occur. However, the clinical value of continuous measurement of MYO for the diagnosis of myocardial infarction is limited by its short duration (*4 h) and lack of cardiac specificity. Therefore, the increase in myoglobin concentration within **8 hours after the onset of chest discomfort in non-diagnostic electrocardiogram patients should not be used as a basis for diagnosis of AMI, but should be supplemented with more heart-specific markers, such as CK-MB , CTnI or cTnT. However, MYO has a high sensitivity, and blood is collected within 4 to 8 hours after the onset. A negative test for MYO can be used to rule out myocardial necrosis. In general, the normal value of MYO is <*8ng / ml, and ≥*8ng / ml is too high, and the risk of acute myocardial infarction (AMI).
Three markers for rapid detection of myocardial infarction: myoglobin MYO, cardiac troponin cTnI, creatine kinase CK-MB. Myoglobin (MYO) and creatine kinase (CK-MB) are most effective for the early diagnosis (<6h) of myocardial infarction (MI), while cardiac troponin (cTnI) is highly cardiac-specific and for the late stage of MI The diagnosis is particularly effective. Because cTnI has the ability to identify patients with mild myocardial damage and has higher diagnostic sensitivity, it is called a mild myocardial injury marker. Although CK-MB is not as sensitive or specific as cTnI, it is still a very useful indicator for detecting myocardial damage. MYO should not be used as the sole diagnostic marker for identifying patients with MI, but its early manifestations are helpful to rule out myocardial necrosis.
