Fill out the appropriate application for your device type and make sure all areas of the application (applicable to your device type) are completed and the application is signed. Submit the nonrefundable application fee and certificate fee as noted on the application. Include operator instruction manuals, installation instructions, pictures and drawings defining the device design. Before submitting an application. Conduct a checklist review for your device using the appropriate checklist available in NCWM Publication *4. Additionally, review NIST Handbook *4 current code requirements for your device. Note: NCWM members receive free of charge (if requested) the current edition of NIST Handbook *4.
